Beverly Hills surgeon reveals shocking truth about dermal fillers and ‘pillow face’ risk

Popular for their “quick fix” appeal, dermal fillers may leave long-lasting—and sometimes permanent—effects, warns top reconstructive surgeon Dr. Kami Parsa.

While many believe fillers are temporary beauty fixes, Beverly Hills surgeon Dr. Kami Parsa says they can last up to 10 years and lead to irreversible facial swelling known as “pillow face.” His viral TikTok video showing filler clumps shocked millions.

In today’s beauty world, dermal fillers are as common as a trip to the salon. With celebrities and influencers promoting them as easy, non-invasive ways to smooth wrinkles or plump lips, they’ve become a go-to for anyone chasing a youthful look. But what if those “quick tweaks” stick around much longer than expected?

Dr. Kami Parsa, a leading oculoplastic and reconstructive surgeon from Beverly Hills, says they do — and the side effects can be startling.

MRI scans reveal the hidden truth

In July 2024, Dr. Parsa posted a TikTok video that quickly went viral, gaining over 12.4 million views. The clip featured MRI scans of a patient who had received over 12 syringes of hyaluronic acid (HA) fillers over six years. Shockingly, the scans revealed nearly 28 cubic centimeters of filler still sitting under the skin — more than double what was injected.

Why? HA fillers are hydrophilic, meaning they attract and hold onto water. Over time, they not only remain in the body but may also expand, causing tissues to stretch.

“You cannot fill your face to youth,” Parsa warns, repeating a line he’s said many times before.

What is ‘pillow face’?

“Pillow face” or overfilled syndrome happens when too much filler is injected into the face, either at once or through frequent touch-ups without allowing old filler to dissolve. The result is an unnatural, puffy, or distorted look that makes people appear older or “worked on,” rather than youthful.

Here’s how it happens:

  • Layering: Adding new filler on top of existing material that hasn’t fully dissolved.
  • Overcorrection: Injecting more than needed in a single session.
  • Skin stretching: Repeated use and filler weight gradually stretch the skin.

The side effects aren’t just cosmetic. Some patients develop chronic swelling or inflammation that may require surgery to fix.

Fillers aren’t always temporary

One of the biggest myths about fillers is that they dissolve within 6 to 24 months. But according to Parsa, traces can remain under the skin for up to a decade, especially with repeated use.

This has sparked concern among users who once trusted the marketing. One user commented that when fillers first became popular, injectors insisted they would only last six months — to which Parsa responded, “That’s what the filler companies were telling us.” Another user expressed regret, saying she got lip fillers once and now believes the substance remains both above and below her lips. Others called the practice frightening, with one person writing that they would never use filler or Botox, describing it as “injecting poison into your face.” Another user added that it was a relief to see this issue finally being discussed, calling out the “mass production and insane usage without warning.”

Less is more

Parsa’s message is simple: Less is more. While fillers can offer a subtle and effective glow-up, overuse carries serious risks. Before making the decision, people should be aware that not all results are reversible — and the effects may last much longer than expected.

What was once sold as a quick beauty fix is now being reexamined under a medical lens. Dr. Parsa’s findings challenge us to ask: At what cost are we chasing youth?
