6 common medications that should never be taken with coffee, experts warn

Pharmacists explain how caffeine can interfere with painkillers, thyroid drugs, and antidepressants.

Many people begin their morning with coffee and a daily medication. Health experts now warn that this routine could cause unwanted or even dangerous effects if the two are combined too closely.

According to pharmacists interviewed by the Daily Mail, caffeine can interfere with how the body processes certain drugs. Pharmacist Budd explained that the liver plays a key role in this interaction. “The main interactions occur in your liver, which can either speed how quickly some medications are broken down or intensify their side effects,” Budd said, advising patients to review when and how much coffee they drink each day. “Even small timing adjustments, like waiting an hour after your dose to have a coffee, can make a difference,” he added.

Below are six categories of medication that experts say should never be taken directly with coffee.

1. Flu and Cold Medicines

Many cold remedies already contain caffeine to reduce fatigue. Others, such as nasal decongestants that include pseudoephedrine, can overstimulate the nervous system when combined with coffee. Budd warned that this mix “can lead to jitteriness, palpitations, or increased blood pressure,” according to the Daily Mail report.

2. Painkillers

Common over-the-counter pain relievers such as paracetamol and ibuprofen can irritate the stomach lining. When taken with coffee—an acidic beverage—the risk of heartburn or stomach upset increases. Pharmacists caution that repeated irritation may lead to more serious conditions, including gastritis or ulcers, if sustained over time.

3. Blood Pressure Medications

ACE inhibitors, which relax blood vessels to lower blood pressure, can be less effective if caffeine is consumed too soon after dosing. As reported by The Daily Mail, caffeine can slow how quickly these drugs are absorbed. This delay may cause blood pressure to remain higher than intended, putting added strain on the heart and arteries. Prolonged stress of this kind can elevate risks of heart attack or stroke.

4. Thyroid Medications

Levothyroxine, commonly prescribed for hypothyroidism, is particularly sensitive to caffeine. Studies cited by Healthline show that coffee can reduce the medication’s absorption by up to 50 percent. The result can be a decrease in effectiveness and the return of fatigue or other symptoms. Health experts recommend waiting at least 30 minutes after taking thyroid medicine before drinking coffee.

5. Antidepressants and Mood Stabilizers

Certain antidepressants, especially selective serotonin reuptake inhibitors (SSRIs), share a metabolic pathway with caffeine. Pharmacist Budd noted that this overlap means coffee can slow how the body clears either caffeine or the antidepressant, increasing side effects such as restlessness or rapid heartbeat. The Finnish health service Duodecim has also warned that excess serotonin—known as serotonin syndrome—can occur if medication builds up in the system, leading to symptoms like sweating, tremors, or confusion.

6. Osteoporosis Drugs

People taking medications for osteoporosis, such as alendronate or risedronate, are advised to take them on an empty stomach with water only. According to Budd, drinking coffee too soon afterward can reduce absorption and blunt the drug’s benefit. He also suggests considering decaffeinated coffee or reducing caffeine overall, since high intake can affect calcium and vitamin D absorption, both essential for bone health.

Health professionals emphasize that not every medication reacts the same way, but timing matters. Patients should always read the medication leaflet and speak with their doctor or pharmacist before combining prescriptions with caffeine. “If you’re ever unsure, speak to your pharmacist or GP,” Budd advised. “They can help you understand whether it’s best to limit anything.”

Moderation, experts agree, remains key—both in caffeine and in confidence when managing daily medications.
