The Tennessee prisons chief has announced his resignation days after the state’s attempt to execute death row inmate Christa Pike ended in a dramatic failure.
According to The Tennessean, Tennessee Governor Bill Lee announced on Saturday, October 3, 2026, that Tennessee Department of Correction Commissioner Frank Strada would step down later this month.
Lee said Strada’s departure was necessary following the failed execution and as an independent review gets underway to establish what went wrong.
“Commissioner Strada has served with integrity, and I appreciate his willingness to put the interests of Tennesseans first during this difficult moment,” Lee said in a statement.
“I have accepted his decision to step down and am grateful for his commitment to ensuring a smooth leadership transition.”
No replacement for Strada has been announced.

Why is the Tennessee prison chief resigning?
Strada, who has led Tennessee’s Department of Correction for about three years, said he was stepping down “in the best interest of Tennesseans.”
He previously served as a senior official in Arizona’s corrections department, where several executions carried out during his tenure encountered complications.
In a statement, Strada defended the department’s handling of Christa Pike’s failed Sept. 30 execution, during which she survived two rounds of lethal injection drugs.
“Serving as commissioner of the Tennessee Department of Correction has been a great honor, and I continue to believe the Department carried out its responsibilities in accordance with the established protocol.”
Further, Strada said he supported the independent review into Pike’s failed execution and would cooperate with efforts to establish what happened.
“I believe this review is entirely appropriate and necessary, and I look forward to doing everything I can to help determine exactly what occurred,” Strada said.
What happened during Christa Pike’s execution?
Pike’s execution was scheduled for Wednesday using pentobarbital, the lethal-injection drug used by Tennessee.
However, the execution did not go as planned.

Pike survived after officials administered two injections of pentobarbital before the execution was ultimately halted. She was later transported from prison to hospital for medical treatment.
Her lawyers said in a court filing on Friday that Pike was intubated and placed on a ventilator while medical staff worked to remove the drug from her system.
The lawyers alleged that problems with the intravenous lines caused the pentobarbital to enter Pike’s body incorrectly rather than being delivered properly into her bloodstream.
“Apparently at no point did any member of the execution team realize that the IV lines were not correctly placed or that the veins had blown and that the pentobarbital was, in whole or in part, entering Ms Pike’s body,” her lawyers wrote in the filing.
Pike’s lawyers react to Strada’s resignation
Pike’s legal team described Strada’s resignation as “justified” but said the move would not undo what happened to their client.
“Strada’s resignation was justified. But it does nothing to help Christa now,” the team said in a statement.
